Lincoln Apps built the ChatGPT for Forms app as a Freemium app. This SERVICE is provided by Lincoln Apps at no cost and is intended for use as is.

App Permissions Explained
The use of information collected through our service shall be limited to the purpose of providing the service for which our Clients have engaged us.
The ChatGPT for Forms add-on requires you to grant permissions to several services associated with your Google Account. It uses the simple and secure OAuth 2 protocol to get data from the user’s Google account and does not have access to your account password.
Here’s a summary of the permissions that the add-on requires and why does it require access to these services.
- See your primary Google Account email address: The app will need to see your primary Google Account email address for the billing purposes, should you decide to use “premium” features
- Display and run third-party web content in prompts and sidebars inside Google applications – The app will need to render its interface using custom-made elements
- View and manage forms that this application has been installed in – The app will need to work with the form’s data (e.g. to pull the results of filling in the form)
- Send requests to 3rd party apps/services – The app will need to communicate with the external API the secure external service built & operated by Lincoln Apps, in order to communicate with OpenAI API
- Read access in Google Drive – To perform its primary function – creating quizzes based on the content of a particular document (e.g., a Google document with a lecture transcript) – the add-on needs to see a list of documents available in your Google Drive, as well as the content of those documents.
Information Collection and Use
For a better experience, while using our Service, we may require you to provide us with certain personally identifiable information. The information that we request will be retained by us and used as described in this privacy policy.
The app does use third-party services that may collect information used to identify you.

ChatGPT for Forms ™ app’s use and transfer to any other app of information received from Google APIs will adhere to Google API Services User Data Policy, including the Limited Use requirements.
The application uses the OpenAI API to generate quizzes. A separate function block in the application allows you to select a file in your Google Drive (e.g. a Google doc) and use its contents to send a quiz generation request to the OpenAI API. If you don’t want to do this, you can choose not to use this feature (generating quizzes based on files in your drive) and use other options, or ultimately uninstall the application.
Cookies
Cookies are files with a small amount of data that are commonly used as anonymous unique identifiers. These are sent to your browser from the websites that you visit and are stored on your device’s internal memory.
This Service does not use these “cookies” explicitly. However, the app may use third-party code and libraries that use “cookies” to collect information and improve their services. You have the option to either accept or refuse these cookies and know when a cookie is being sent to your device. If you choose to refuse our cookies, you may not be able to use some portions of this Service.

Children’s Privacy
These Services do not address anyone under the age of 13. We do not knowingly collect personally identifiable information from children under 13 years of age. In the case we discover that a child under 13 has provided us with personal information, we immediately delete this from our servers. If you are a parent or guardian and you are aware that your child has provided us with personal information, please contact us so that we will be able to do the necessary actions.
